[edit] Benign Infantile Focal Epilepsy with Midline Spikes and Waves during Sleep

Capovilla et al[1] described the electroclinical features of 19 infants with focal seizures and typical midline sleep EEG abnormalities and benign outcome.Seizure manifestations were typical, characterized by cyanosis, staring and rare lateralizing signs, of short duration. Age at onset was comprised between 4 and 30 months. The typical EEG marker, a spike followed by a bell-shaped slow-wave, localized in the midline regions, was present in all subjects only during sleep. All infants had favorable outcome and majority were not treated.

Flesler et al[2] analyzed the electroclinical features and evolution of seven infants with benign infantile focal epilepsy with midline spikes and waves during sleep (BIMSE). The onset of seizures were between six and 13 months of age (mean, 10.2 months; median, 11 months).  All patients suffered from seizures during wakefulness and seizure manifestations consisted of staring, stiffening, cyanosis, automatisms and lateralising signs. The seizure duration ranged between 30seconds and 5 minutes. The interictal EEG recording during sleep showed low-voltage unilateral or bilateral spikes located in the central and vertex regions, followed by slow waves in all patients. All patients had a favourable outcome

It has been suggested that Benign Infantile Focal Epilepsy with Midline Spikes and Waves during Sleep (BIMSE) is a new syndrome rather than an early presentation of benign epilepsy of childhood with centrotemporal spikes, Panayiotopoulos syndrome, or a late presentation of benign focal infantile seizures. Benign Infantile Focal Epilepsy with Midline Spikes and Waves during Sleep (BIMSE)is not currently included in the ILAE 2009 classification.
