Comparative study of Febrile seizures Vs Generalised epilepsy with febrile seizure plus syndrome

Abstract


Introduction: Generalizedepilepsy with febrile seizure plus (GEFS+) described as  autosomal dominantinherited epilepsy disorder. Currently no literature available from India.  Aims& objectives: To study and compare seizure profile andneed for anti epileptic drugs(AED) in febrile seizures versus GEFS group attertiary care pediatric center. Study design and setting:Prospective observational study, Non interventional study.Results: Total 176 childrenenrolled in study, 75 were under GEFS group and 101 under Febrile Seizuregroup.Mean age of onset at 1stseizure was 17.9 months (± 12.45) in GEFS group and 19.87 months (+ 11.77) in Febrile seizure group.  Male: female ratio  1.58:1 in GEFS group and 1.72:1 in Febrile seizure group. 45%  had first febrile convulsion below 12 monthsv/s 21% in Febrile seizuregroup. Longer duration of seizure >5minutes seen in GEFS group than febrileseizures(32% vs 17%). Recurrence  in 52% of GEFS Vs 16%  Febrile seizure group. Recurrence of febrile and afebrile seizure 48% and 24% in GEFS, 15% and 1% in Febrile seizure. AED  required in 33% of GEFS groupv/s 1% of febrile seizure. AED (1 yr follow up) continued in 24% of GEFS  v/s 1% Febrile seizure group.  Neuroimaging and EEG were poor predictors of recurrence.Speech and behavioral issues noticed in 4% of GEFS, none in Febrile seizuregroup.Conclusion: Childrenwith GEFS plus had higher incidence of febrile & afebrile seizuresrecurrences, higher need of AED 33% v/s 1% in febrile seizure group. Speech andbehavioral issues noted in GEFS group.
