Gratification phenomena in infancy: a report of twenty-nine children.

Abstract


Objectives: To report a series of children with different patterns of gratification phenomena seen in Oman.

Introduction: Gratification phenomena (disorder) also known, as “benign idiopathic infantile dyskinesia” is a form of non-epileptic abnormal movement disorder seen in early infancy.1 It occurs in children between 3 months to three years. These movements are masturbatory behavior that is often mistaken for epilepsy (infantile spasms), dystonia, dyskinesia or even abdominal pain.2 This is a benign event and a normal behavioral variant seen in early childhood.

Methods: All the children with the diagnosis of gratification phenomena seen in last ten years were analyzed. A proper history, eyewitness account and video recording of the event helped in the diagnosis.

Results: 27 of twenty-nine (93%) children were females. Age at onset was 3 months to 2 years one month with a mean age of nine months. 27 out of 29 (93%) had onset less than twelve months. Four patterns of gratification were seen. Three of these patterns were not described before and are reported. A typical gratification phenomenon was seen in 16 of 29 (55%). At the time of referral 8 (27%) were labeled epileptic and were on antiepileptic medication.

Discussion: Gratification phenomena is a benign phenomena, often labeled as epilepsy. Proper history taking is essential for making diagnosis. Videos of the event are diagnostic.  Reassurance and explanation is the main management. These children are investigated for epilepsy and prescribed antiepileptic medications.

Conclusion: We have to recognize this benign condition, avoid extensive investigations and the children are not exposed to antiepileptic drugs.
