case series of pediatric multiple sclerosis

Abstract


1. Introduction: Multiple sclerosis is an inflammatory disorder of the central nervous system targeting the myelin.Pediatric multiple sclerosis is rare and there is scanty literaturefrom India.Majority  of these patients present as clinically isolated syndrome  and are lost to follow up by the pediatrcians as they grow up into adulthood.2. Methods: The current study is a description of  pediatric patients with multiple sclerosis It is an observational study over a period of 8 years.Longitudinal follow up of these patients was conducted to study the progression of the disease and the disability.All  the patients consented for the study.Details pertaining the demographics and history regarding the clinical symptom(optic neuritis,brainstem ,spinal cord syndrome ) and the number of relapses were documented.The disability was scored by Kurtzke’s expanded disability scale(EDSS).Radiological features based on Magnetic resonance imaging(MRI) of brain and spine including those obtained after gadolinium administration were obtained.Patients then underwent cognitive testing using scales for individual cognitive domains. .Psychosocial impairment was documented by  Hospital anxiety and depression scale.The treatment used and the response to therapy was documented.3. Results: Descritive study of 5 patients in the adolescent group.Four of them are doing well.one has severe disability and has converted to progressive MS and is currently non ambulatory.Under detailed analysis.Conclusion/Discussion: Pediatric MS is rare.The progression is variable.Majority of them do well .Cognitive and psychosocial disturbances cause a greater limitation than the physical disability
