ALERD In a Post-Operative Case Of a 20 Month Girl Child.

Abstract


ALERD (ACUTE LEUKO ENCEPHALOPATHY WITH RESTRICTED DIFFUSION) is a syndrome characterized by seizures, fever and altered consciousness in the acute stage followed in the subacute stage by restricted diffusion in the subcortical white matter on magnetic resonance imaging . Most of them are caused by viral infections.

We here report a 20 month girl developed ALERD  features during the post-op (ARM) period on Day-5 and without biphasic pattern of seizures generally seen in AESD. Child was treated with AED’s . At 3months  follow-up , child was cognitively normal and seizure free. Repeat  EEG after 6 months was normal & AED’s were tapered.

Whether ALERD was coincidental in the post-op patient or the stress of surgery precipitated ALERD needs to be studied further.
